REHABILITATIVE PSYCHOSOCIAL SERVICES | COMMUNITY INTEGRATION | PROGRAM PLAN
Psychosocial Rehabilitation provides treatment to adult clients diagnosed with a serious mental illness. Psychosocial Rehabilitation services are designed and provided to the clients in order to promote recovery, restore skills and develop adaptive behaviors necessary to sustain the individual in the community.
Psychosocial Rehabilitation Services are individualized and relates to the clients strengths, needs, abilities and preferences. These services assist the clients with eliminating functional deficits and interpersonal barriers created by their illness.
The program serve individuals ages 18 and over with a serious mental illness (with or without co-occurring disorders) that impair functioning in daily living such as living arrangements, personal relationships and employment.
Persons with a serious mental illness may be eligible for admission to the Psychosocial Rehabilitation Program. Referrals can be made by the treating mental health physician, case manager and/or a self-referral by the client.
HOURS OF OPERATION
The Psychosocial Rehabilitation program is in operation 8:00 A.M. � 1:30 P.M. Monday through Friday. This program is closed on DMH approved Holidays.
AFTER HOURS CONTACT
Emergency after-hours services are available to the clients in the Psychosocial Rehabilitation Program. The client should call the main phone number to the mental health center in their county for assistance after hours. Clients in the Psychosocial Rehabilitation Programs are provided with a wallet sized laminated card that details the afterhours emergency contact phone numbers.
Psychosocial Rehabilitation services are provided in each county served, in their own designated locations. These settings provide room for group discussion and personalized treatment when necessary.
We strongly believe that the clients have the right to develop personal growth, to gain commitment to community integration, to have access to goal oriented and individualized supports and to become empowered to seek satisfaction and success in community living.
FEES AND PAYOR SOURCES
The Department of Mental Health accepts major payer sources including Medicaid, Medicare and Private Insurance. During the administrative intake, individuals are provided a copy of the current rate schedule. At any time individuals may contact their local clinic to ask the administrative specialist current rate schedules, or to determine insurance coverage.
The goal of Psychosocial Rehabilitation is to stress the person�s strengths to promote increased stability and to improve their quality of life. Additionally, treatment interventions are provided to prevent hospitalizations. Objectives include reduced psychiatric symptoms, improved ability to function as part of the community and increased community tenure. Outcome measures include GAF scores, Client Satisfaction Surveys, AOP MHC Housing/Employment reports, Community tenure statistics /Living arrangement statistics and State psychiatric hospital admission/discharge reports.
MECHANISMS TO ADDRESS NEEDS
The services provided to the clients are planned and structured. Services may be provided on a short term or long term basis.
Services include but are not limited to the following
� enhancement of the client�s understanding of and ability to cope with his/her psychiatric disability
� communication skills
� goal setting
� self care and personal hygiene
� appropriate eating habits and selection of food according to any diet restrictions
� management of living space and upkeep of personal belongings/property
� rational problem solving
� medication use and symptom management
� time management
� stress reduction
� work readiness skills
� advocacy services
� co-occurring alcohol and drug treatment
� job placement
� linkage to medical services outside of mental health
INTERDISCIPLINARY TREATMENT TEAM
Psychosocial Rehabilitation programs are staffed with a
mental health professional and bachelor level staff. Staff with backgrounds in
nursing, counseling, rehabilitation and recreational therapy, assist the clients
in accomplishing their goals.
